Concierge Home Care is hiring an Assisted Living Liaison (LPN) to serve as the connection between hospital teams, physicians, patients, and home health services. This role bridges the gaps in transitional care.
Job description
Key Responsibilities
- Build relationships with executive directors, social workers, physicians, and wellness directors
- Educate referral sources on home health services, programs, eligibility, and Concierge Home Care offerings
- Support referral growth and admissions goals
- Meet patients at bedside prior to discharges
- Explain home health services and benefits
- Educate on insurance coverage, eligibility, and skilled home care services
- Advocate for safe and seamless discharges
- Coordinate care from hospital to home
- Collaborate with DME providers, infusion companies, PCPs, and caregivers
- Support patients during critical post-discharge periods
- Help reduce avoidable readmissions
- Partner closely with intake teams, clinical managers, schedulers, and account executives
- Provide timely referral and patient updates
- Ensure continuity of care throughout the admission process
- Process referrals in real time
- Complete admission documentation accurately and in a timely manner
- Track referral activity, conversion rates, and admission barriers
Qualifications
- Active Florida or compact LPN license
- Strong communication and interpersonal skills
- Detail-oriented and organized
- Microsoft Office proficiency
- Valid driver’s license and reliable transportation
- Auto insurance
Preferred
- Home health experience
- Assisted Living experience
- Case management or transitional care experience
- Knowledge of payer sources and referral processes
