The Opportunity
Abby Care is launching in the greater Phoenix market and building its founding team. As the founding Case Management Coordinator, you will own key relationships with health plan partner case management leaders, supervisors, and individual case managers, making referring to Abby Care simple and obvious. You will also support provider relationships in the early days.
What you'll work on
- Build & Own Relationships: Be the main point of contact for health plan partner's case managers, from leaders and supervisors to individual case managers.
- Start With Leaders: Map the case management organization, find a champion on each team, and plan how and when to reach their teams.
- Consultative Education: Teach who fits the program, how to refer in minutes, and what happens next. Run team sessions and a launch roadshow of in-person events.
- Keep Case Managers in the Loop: Acknowledge every referral the same business day, send updates on every family, and get the care plan to the payer case manager when done.
- Own Authorizations: Request, track, and chase authorizations from submission through approval and renewal.
- Support Providers: Help PCP offices with records, status updates, and stalled requests in the early days.
- Track Everything: Log every interaction, referral, and authorization in the CRM and use it to see where a relationship needs attention.
What you'll have
- 3+ years of relationship, care coordination, provider relations, or business development experience, ideally with a health plan, a case management team, or home- and community-based care.
- A feel for how case managers think and what they need to place a family with confidence.
- Familiarity with the Arizona Medicaid landscape is a big plus.
- A consultative edge.
- Great written and verbal communication.
- Organized to a fault.
- A founding mindset: you have built a relationship, a program, or a process from zero.
