Company overview
Community Health Partnership of Illinois (CHP) is a Federally Qualified Health Center (FQHC) established in 1991 that provides comprehensive, culturally responsive healthcare services to medically underserved populations across northern and central Illinois. As a 501(c)(3) nonprofit organization, CHP operates six health and dental clinics along with multiple outreach sites serving communities in Aurora, Harvard, Kankakee, Mendota, Champaign, Princeville, and Rantoul. The organization delivers integrated medical, dental, and behavioral health services with a patient-centered approach, serving over 10,000 individuals annually. CHP's mission focuses on empowering uninsured, underserved, migrant, and seasonal agricultural workers to attain optimal health regardless of their ability to pay, with 99% of patients coming from low-income households and three-quarters lacking health insurance. The organization employs award-winning Promotores de Salud and bilingual-bicultural outreach teams to conduct health screening and service navigation, while also participating in the 340B Drug Pricing Program to provide affordable medications to vulnerable populations.
