The Discharge Planner provides discharge planning and case management services, crisis intervention and liaison services with referral sources and other departments within the hospital in a manner consistent with the philosophy and objectives of Lakeside Behavioral Health System.
Responsibilities
- Provide discharge planning and case management services
- Provide crisis intervention
- Provide liaison services with referral sources and other hospital departments
- Demonstrate flexibility for reassignment to units providing care to varied populations including adults and dually diagnosed patients in inpatient and partial hospital settings
Qualifications
- Bachelor's degree in Social Work, Clinical/Counseling, or Social Science related area required
- Licensure preferred
- Graduate of an accredited nursing program
- Prior experience working with a psychiatric mental health population, preferably in an inpatient setting
- If a Registered Nurse, must have licensure and registration by the Tennessee Board of Nurse Examiners as a registered nurse
- Prefer knowledge of case management, discharge planning, family therapy theory and practice
