Job description
Essential Functions
- Provide mental health assessment to determine risk and treatment needs in a hospital setting
- Coordinate appropriate level of care
- Obtain collateral information
- Determine inpatient, Crisis Stabilization, outpatient, and mobile services
- Determine and implement discharge plans, secure referral appointments, and facilitate all necessary lines of communication
- Responsible for accuracy and completion of all documents
- Maintain records and data entry
Requirements
- Master's degree in social work, marriage and family therapy, professional counseling, psychology, or 4 year Registered Nurse degree certified in psychology and 2 years mental health experience
- Must meet legal criteria for Qualified Mental Health Professional
- Kentucky licensure to practice (LCSW, LPCC, Psychology Practitioner, Licensed Psychology Associate, or certified psychologist)
- Two to three years' experience providing case management to individuals with mental health illness or addiction
- Must have demonstrated experience developing, conducting assessments, individual service plans, treatment plans, and documentation abilities
