Requirements

• Current, active, and unencumbered Licensed Practical Nurse (LPN) license to practice in the State of Georgia
• Current Basic Life Support (BLS) certification
• Experience providing care to medically fragile individuals
• Working knowledge of applicable home and community-based service programs, including ICWP, CCSP, SOURCE, and GAPP
• Ability to provide nursing services within the LPN scope of practice and in accordance with established Plans of Care
• Ability to travel to client homes and other service locations within the assigned territory
• Maintain automobile insurance coverage that meets applicable state and company requirements throughout employment
• Previous home health, home care, or community-based nursing experience
• Experience providing skilled nursing services in the home setting
