Summary
Nexus Family Healing is hiring a part-time Licensed Practical Nurse for the Southeast Regional Crisis Center (SERCC) in Rochester, MN. The LPN provides medical services, procedures and protocols that promote resident health and well-being.
Job description
Primary responsibilities
- Provide client-centered medical care and monitor acute and chronic health conditions.
- Assess, treat, and respond to medical concerns and emergency situations, including coordinating referrals when needed.
- Conduct nursing assessments and support the admissions process.
- Maintain accurate and up-to-date medical records and documentation.
- Collaborate with physicians, treatment teams, and interdisciplinary staff to support client care plans.
- Coordinate follow-up appointments, referrals, and ongoing medical services.
- Serve as a medical resource for staff and contribute to individualized treatment planning.
- Administer, monitor, and document medications in accordance with policies and procedures.
- Educate clients on medications, including potential side effects and interactions.
- Ensure compliance with confidentiality, privacy, and healthcare regulations.
- Maintain required licensure, certifications, and role-specific training requirements.
- Promote client health, wellness, and positive treatment outcomes.
Required Education and Licensure
- Graduate of an accredited school of Practical Nursing
- 2+ years' experience in a clinic and/or hospital environment
- Valid driver's license required. Must meet state regulating agency and Nexus Home Office driving requirements
- Must successfully pass a DHS criminal background check.
- Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
Preferred Education and Experience
- 4+ years' experience in a clinic and/or hospital environment
