DaVita is looking for a PER DIEM Licensed Social Worker (LISW) (LICSW) (LGSW) to support our patients and their families at our Cass Lake Dialysis Clinic in Cass Lake, MN.
Job description
What you can expect
- Covering Cass Lake Dialysis clinic only
- Will need to be in clinic at least twice a month
- Per Diem status - fewer than 24 hours a week
- Training - 6-8 weeks
- 50 patient case load
- Will provide group supervision 4 hours a month that is no cost to the teammate

Requirements
- Licensed or registered in the state of practice as required by state regulations -- (LISW) (LICSW) (LGSW)
- Meets all state required regulations to practice in the dialysis setting
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
