Primary Responsibilities
- Promote whole-person integrated care by planning, coordinating, tracking, and closing gaps in care.
- Monitor care to individuals and achieve key goals of the tailored plan related to behavioral health, mental health, and physical health.
- Comfortable with direct patient contact, including in-home and community appointments.
- Experience with NC Tailored Care Management is required.
The Care Manager shall report to the Care Manager Supervisor. This is a hybrid position with some in office.
Requirements
- Minimum of a 4-year degree within Human Services
- Minimum of 2 years full-time experience
- Experience with NC Tailored Care Management
- Qualified professional (not licensed or provisionally licensed)
- Comfortable with direct patient contact, including in-home and community appointments
